I understand that this card is not transferable and must be presented before library materials may be lent or other services obtained. I agree to return this card should I leave the area served by the library and that the card remains the property of the library.The information on this form is collected under the authority of the Libraries Act and the Freedom of Information and Protection of Privacy Act. The information provided will be used to issue a library card, contact you about requested or borrowed library resources, calculate administrative statistics and provide you with information on library programs and services.
